I've always written it as Frank Black and the Catholics, but I've also seen it as Frank Black & The Catholics, or Frank Black and The Catholics. Which one is correct? I've always assumed the band was simply known as Catholics, so adding a the is simply for aesthetics (like the Pixies).

iTunes has it down as Frank Black & The Catholics, though.

It really annoys me when iTunes doesn't group the Frank Black under one title.

I would go with the "and"

What about (the/The) Pixies? What's the actual name of 'em?

Pixies/the Pixies/The Pixies

Plain Pixies is just fine Aurora.

But it does get confusing when writing Pixies into a sentence.
Sometimes it's grammatically correct to place a 'the' before the word.

It's the Catholics, not The Catholics. FYI. (can't tell if anyone actually ANSWERED you)

If I remember well, there once was one of the catholics who used to post here (who was it? Rich?), and Dave gave him a title of "Catholic" and he asked that the uppercase was removed ("catholic" then). I don't remember the exact details though.
