I would personally go with Bumbles idea of Iain banks books...but chech he doesnt already own them!!!
Also, maybe you coudl buy like a gothicky corset and stokcings and stuff, and just go 'ta da, happy birthday' (this would usually work best if you were actually wearing the clothes....)

the best gift i could imagine would be a good book, a good album and a good six pack of some special beer. if this is someone you see yourself spending $50 or so, you should be able to get those things. if you're on more of a budget, you can't go wrong with a good book.

If he's bald, or has a shaved head, I'd buy one of those old Wooly Willy toys, where you draw on hair using magnets and metal shavings, and then replace the target drawing of the face with the guy's photograph. Atta be a hoot! Silly, fun, thoughtful gifts are the best.
Either that, or a copy of On the Road. Everyone should have a copy of On the Road.
