I hate the doctors, and have never really been, so I have no idea what a doctor note looks like, if its done on particular paper, if theres certain wording or anything!!!

Posted - 11/03/2004 : 06:33:09 Won't work, they're all standard format official paper jobbies, if doctors freak you out most GP offices have a nurse practitioner you can ask to see instead, they write them too sometimes

Posted - 11/02/2004 : 17:28:28 I wrote plenty of notes when I worked for an OB/GYN . The key is to have it on their letterhead. I always wrote them on prescription pads. Buddy up to a nurse, that's my advice.

Posted - 11/02/2004 : 16:45:50 I'm afraid they are of standard format in a mostly tick the box type style with room for notes at the bottom and the doctors name and number on it, most uni's/colleges check up too.
Your best bet is to go see a doctor, make up symptoms of stress and claim embarassment at seeking help before hand and they will write a retrospective note in most cases.

Posted - 11/02/2004 : 16:05:40 It's usually on an offical-looking paper that came from a pad, about 4" x 6". Doctors love scribbling things on pads of paper. It should have this doctor's name and title at the top, like a letterhead, in some impressive- looking gothic all-caps font. I suggest you make up a bogus title. |

Posted - 11/02/2004 : 04:58:27 the most important thing is to make it unreadable. if it can be read by anyone that is not a doctor or a pharmacist they will immediately understand that it's a fake. and at least over here(but most probably in every country) doctors have to use specific modules to prescribe or to prove a visit.
