i ran across the same problem when i got it, and couldn't find anything, so i made my own.
and here it is:

it's in photoshop format, so hopefully that's ok.

i don't have photoshop so i made my own,i'd post it here but don't know how to get a url for my image

if you use coverxp to print covers (and you should - it's very useful!), it will be able to use photoshop files.
